Amputee satisfaction survey
We appreciate you and your opinion. To help us improve our services, please consider taking this short survey to let us know how you feel about your treatment at NZALS.
The survey is anonymous and takes about 5 minutes to complete.
Making a complaint
If you are dissatisfied with your treatment as an amputee, whanau/family or caregiver, you have the right to lodge a complaint regarding the service provided by the NZALS which can include:
1) Treatment by any NZALS team member.
2) The provision of any service in any limb centre
3) The provision of an artificial limb
4) Issue to relating to how their personal or health information has been collected
